What to Send

The following check-list is a guide for packaging all pertinent items.

• Final impressions
• Copy of ‘Smile Analysis Form’.
• Photos. 
• Diagnostic cast, conveying incisal edge positions and proper functional surfaces. This cast should be the blueprint for the final restorations.
• Centric bite registration.
• Counter-cast or impression.
• Rx form, which should contain at least the following:
1. Stump shade for all Empress cases.
2. Final shade
3. Names of Doctor and patient.
4. Due date: Cases received at the lab without a due date will be automatically scheduled for about two weeks.
5. Designate male or female.
6. Doctor’s signature.
7. All additional pertinent technical information.
